Pick from this list of documents below, and send us one copy of your identification, a copy of your social security card, and proof of your address. You can send your information to us through the client tracking portal, fax, or mail.
Here is the list of documents to pick from. Make sure the identification copies are clear and readable. For ID’s, we should be able to see your face.[/text_block]

- Driver’s license

- Current utility bill
- Bank statement
- Insurance statement
- Copy of your social security card

- Cancelled check
- Government-issued ID card
- Signed letter from homeless shelter
- Stamped post office box receipt

- Valid Driver’s License
